How can the Nursing team help?

The Nurses helps you manage your health and wellness through lifestyle changes.

If you are struggling with a chronic issue or disease, you may be referred to an Nurse for additional support and treatment. Our nurses coordinate care, coach, educate, and advocate for you. Services include:

Providing information on prevention and self-management, including:

• Lifestyle change and health management
• Medical conditions
• Medication education
• Medical equipment, such as Glucometers
• Classes for adults and children on topics such as weight loss, diabetes, and smoking cessation
• Stress management and coping
• Screening for functional limitations, fall risk and other health risks
• Addiction

Providing information regarding services based on the needs of patients and their families. Nurses assist with obtaining community resources and coordinating care while considering the patient’s cultural needs and expectations. This includes:

• Providing support and resources for family caregivers in managing care at home
• Informing patients of local food pantries and charitable resources
• Directing patients to safe havens when in abusive environments
• Registering for emergency assistance
• Providing information on travel clinics for appropriate immunizations
