Providing information on prevention and self-management, including:

Medical equipment, such as Glucometers

Classes for adults and children on topics such as weight loss, diabetes, and smoking cessation

Stress management and coping

Screening for functional limitations, fall risk and other health risks

Providing information regarding services based on the needs of patients and their families. Nurses assist with obtaining community resources and coordinating care while considering the patient’s cultural needs and expectations. This includes:

Providing support and resources for family caregivers in managing care at home

Informing patients of local food pantries and charitable resources

Directing patients to safe havens when in abusive environments

Registering for emergency assistance

Providing information on travel clinics for appropriate immunizations
