How can the Nurse Care Manager help?

The Nurse Care Manager (NCM) helps you manage your health and wellness through lifestyle changes. If you are struggling with a chronic issue or disease, you may be referred to an NCM for additional support and treatment. The NCM coordinates care, coaches, educates, and advocates for you. NCM services include:

Providing information on prevention and self-management, including:

  • Lifestyle change and health management
  • Medical conditions
  • Medication education
  • Medical equipment, such as Glucometers
  • Classes for adults and children on topics such as weight loss, diabetes, and smoking cessation
  • Stress management and coping
  • Screening for functional limitations, fall risk and other health risks
  • Addiction

Providing information regarding services based on the needs of patients and their families. NCM’s assist with obtaining community resources and coordinating care while considering the patient’s cultural needs and expectations. This includes:

  • Providing support and resources for family caregivers in managing care at home
  • Informing patients of local food pantries and charitable resources
  • Directing patients to safe havens when in abusive environments
  • Registering for emergency assistance
